Preventing Hospitalisation

Course Description

Hospitalisation is common among aged care residents, with the most frequent causes including respiratory illnesses, injuries from falls, circulatory conditions, and other acute medical issues. Many of these hospitalisations may be avoidable if risk factors such as inadequate falls prevention, poor nutrition, medication mismanagement, or delayed recognition of health decline, are properly addressed within the aged care setting.

Hospital transfers can have serious negative outcomes, including increased risk of complications like hospital-acquired infections, delirium, mobility decline, and pressure injuries, as well as emotional distress, especially for residents with cognitive impairment. They also place significant demands on healthcare resources and may impact organisational compliance and reputation. Therefore it is important for care workers to be aware of key risk factors for hospitalisation to maintain resident health, safety, and quality of care.

Learning Outcomes

To equip care staff and teams with the knowledge, skills and strategies to proactively identify, assess and manage risk factors such as falls, nutrition issues, infections, and care transitions, thereby reducing avoidable hospitalisations and improving resident safety, stability, and quality of life.

Apply evidence based prevention strategies, including falls prevention, nutrition and hydration management, pressure injury prevention, medication review, infection control, and safe mobilisation.

* Support transitions effectively to minimise disruption in care and reduce risks such as decline in health status and readmissions
* Develop care plans tailored to individual resident needs and risk profiles , integrating regular monitoring, early intervention, and multidisciplinary input.
* Enhance team coordination and communication to ensure timely detection and response to changing resident needs.
* Track and review resident events (falls, weight loss, pressure injuries, hospital transfers) to continuously refine care practices and reduce avoidable admissions.
